Thank you for choosing to join M.M.D.N.A. - the procedure is simple and fast.

To join M.M.D.N.A.:

1. Download a copy of the application form (click on icon below) and unzip it.
   

2. Print it and Fill it in,
   

3. Post it to M.M.D.N.A. - Ingham House. 91,93 Triq ir-Russet ,San Gwann, SGN 4434, Malta GC
   

4. A membership card will be sent by post.
   

Join M.M.D.N.A.  - Categories & Forms

On registration a member is provided with a copy of the Membership Regulations.

There are two categories of members:

1. Individual -  a person who applies in his own name. Membership includes the Member and his wife or her husband as well as other dependents (under 18 years) of the Member and living in the same household.
   

2. Group -  person forming part of a group of 25 or more. Membership applies only to the Member.
   

In addition to nursing service the following additional benefits are available to Members:

1. Provision free of charge of syringes (Excluding to diabetic patients), Sterile dressings packs, swabs and tubular stretch bandages for the first month. After which these can be bought at cost price.
   

2. Visits by appointment for which a fee of Euro1.00 is payable
   

Fees payable annually are:

1. Individual - Euro 23.29 on joining and Euro 23.29 per annum membership fee. Thereafter an annual fee of Euro 23.29.
   

2. Group - Euro 2.33 per member for a group of 25 or more on joining, Euro 2.33 per member thereafter. Euro 58.23 for a group of less than 25.

Membership Renewal

You can renew your subscription in an easy way. Once you complete the two forms below, your individual subscriber's card will be posted to you by 1st January each year.

Renew your membership to M.M.D.N.A.:

1. Download a copy of the 2 forms (click on icon below) and unzip them.
   

2. Print them and Fill them in,
   

3. Post 'Renewal M.M.D.N.A.' to M.M.D.N.A. -  Ingham House. 91,93 Triq ir-Russet ,San Gwann, SGN 4434, Malta GC
   

4. Hand in 'Renewal Bank' to your bank.
   

5. A membership card will be sent by post.
